Overview

What is HSE audit software?

HSE audit software runs internal and external audits as structured digital workflows. Auditors record findings and evidence against each audit question, every finding is linked to a corrective action with an owner and a deadline, findings and closures go through an approval path, and the audit report is generated from the records instead of written by hand.

SparkFlow works as HSE audit software for the whole audit cycle: the HSE manager sets up the audit, auditors record findings in the field or the office, action owners close what was found, and leadership reads the result on a live dashboard. Audits sit on the same platform as your inspections, incidents, permits and risk records, so the evidence an auditor samples is already connected.

Audit types

Internal, client and supplier, and third-party audits on one record, so every finding follows the same path.

Audit criteria

ISO 45001, ISO 14001, ISO 9001, legal requirements and your own procedures, set up as configurable audit forms.

What you get out

Findings with evidence, corrective actions with owners, approvals, dashboards and audit reports from live records.

Audit vs Inspection

HSE audits vs safety inspections: what is the difference?

The two are often treated as one activity, but they answer different questions. An inspection asks whether a place, asset or task is safe right now. An audit asks whether your management system is working, and whether you can prove it.

HSE audit compared with safety inspection
Comparison point Safety inspection HSE audit
Question it answers Is this site, asset or task safe right now? Is our HSE management system working as intended?
What it looks at Physical conditions and behaviour at a point in time Processes, records and evidence over a period, compared with criteria
Measured against A checklist of observable items A standard such as ISO 45001, a legal requirement or your own procedure
Typically done by Supervisors, HSE officers, contractors Competent, impartial internal or external auditors
Output Findings and corrective actions Nonconformities, observations, opportunities for improvement and an audit report

Inspection records are one of the things an auditor samples. If you want to digitise the site-level checks, see safety inspection software. Both run on the same SparkFlow records, so findings from either close through the same action process.

Audit Types

Internal, external and third-party audits

Audits are usually described by who carries them out. Each type produces findings that need the same follow-through, which is why one record for all of them matters.

Internal audits

Carried out by or for your own organisation to check your management system against your procedures and standards such as ISO 45001. Often called first-party audits.

Client and supplier audits

A client auditing its contractor or supplier, or your team auditing the contractors who work for you. Often called second-party audits.

Certification and regulator audits

Carried out by an independent body such as a certification body or a regulator. Often called third-party audits.

Why Go Digital

Why spreadsheets and documents let audits down

With documents and spreadsheets

  • Audit notes are written on paper or in Word and typed up days later.
  • Findings live in a spreadsheet that is separate from the evidence behind them.
  • Corrective actions are chased by email, so overdue items go unseen.
  • Nobody can say quickly whether last year's findings were closed.
  • The audit report is assembled by hand from several files.

With SparkFlow

  • Findings and evidence recorded as the audit happens, with no retyping.
  • Each finding stays linked to its evidence, its action and its closure.
  • Actions have owners and deadlines, and overdue items show on the dashboard.
  • Open and closed findings from earlier audits can be found in seconds.
  • Audit reports generated on demand from the live records.
Audit Workflow

From audit set-up to closed findings: the audit lifecycle

An audit does not end when the auditor leaves. Each stage below is tracked in SparkFlow, so a finding cannot fall between the person who raises it and the person who fixes it.

  1. Set upConfigure the audit form, scope and approval path for your hierarchy.
  2. RecordCapture findings and evidence against each audit question.
  3. Link actionsRaise corrective actions with an owner and a deadline.
  4. ApproveSend findings and closures through your approval path.
  5. ReportGenerate the audit report from the records and review results.

Why follow-up matters: ISO 45001 clause 9.2 expects internal audits at planned intervals, run to an audit programme with defined criteria and scope, by impartial auditors, with results reported to management and action taken on nonconformities. ISO 19011 gives guidance on managing an audit programme and carrying out audits. A system that links each finding to an action makes that evidence a by-product of the work.

Traceability

One finding, one complete trail

When an auditor asks how a finding was handled, the answer should be a single record, not a hunt through email, spreadsheets and photo folders. In SparkFlow the audit question, the finding, its evidence, the action and the approval stay linked from the day the finding is raised.

  • Start from the audit question so every finding is tied to a requirement.
  • Keep evidence on the finding instead of in a separate folder.
  • Show who owned the action and when it was due.
  • Prove closure with the approval recorded on the same record.
Capabilities

What SparkFlow HSE audit software does

Everything an HSE team needs to move from an audit question to a closed finding, on one platform.

Configurable audit forms

Build audit checklists around your own standard or procedure, not a generic template. Forms, routing and approval paths follow your hierarchy and departments.

Mobile audit capture

Record findings, observations and photo evidence on site from web and mobile, instead of writing notes and typing them up later.

Findings with evidence

Attach evidence and observations to each finding, so the record shows what the auditor saw, where and against which question.

Findings linked to actions

Turn any finding into a corrective action with an owner and a deadline, tracked to closure in Risk & CAPA, so nothing sits in a report.

Learn more

Approval workflows

Route findings and closures through the approval path that matches your hierarchy, so sign-off is recorded on the same record.

Audit reports on demand

Generate audit-ready reports from the records, rather than compiling findings from several files the night before.

Live dashboards

See open findings, actions and their owners across sites in one view, so management review works from live data.

Connected HSE records

Link audits to inspections, incidents, permits and risk assessments on one platform, so the records an auditor samples are already connected.

Audit evidence should be a by-product of the work, not a project the week before

When findings, evidence and actions are recorded as they happen, preparing for an audit becomes a review of live records instead of a scramble through folders.

  • Recorded as it is found
  • Owned and dated
  • Approved on the record
  • Reported from live data
Audit Findings

Nonconformities, observations and opportunities for improvement

Audit findings are usually graded so the response matches the seriousness. These are the categories most organisations use. Definitions and thresholds vary, so follow your own procedure or the rules of your certification body.

Common audit finding categories
Finding What it usually means Example
Major nonconformity A requirement is missing or has failed in a way that puts the effectiveness of the system in doubt. No process exists to evaluate compliance with legal requirements.
Minor nonconformity An isolated lapse against a requirement that does not undermine the system as a whole. One contractor's induction record cannot be found.
Observation Something that meets the requirement today but could become a nonconformity if left alone. Drill records are kept but nobody reviews them.
Opportunity for improvement A suggestion that goes beyond the requirement and would make the system work better. Merge two similar permit forms to cut duplication.

Forms in SparkFlow are configurable, so the finding categories on your audit form can follow your own procedure. Whatever the category, every finding that needs action is linked to a corrective action with an owner and a deadline.

Follow-Through

Why audit findings keep coming back

Repeat findings are rarely caused by careless people. They come from gaps in follow-through. These are the six most common, and how recording the whole trail in one place closes each gap.

Fixing the symptom, not the cause

A missing record is replaced, but the process that lost it is left alone, so the same finding returns next year.

With SparkFlow: raise a corrective action against the cause, linked to the original finding.

No clear owner

An action assigned to "the site" or "the team" belongs to nobody, so nobody chases it.

With SparkFlow: every action has a named owner on the record.

Deadlines that slip quietly

Dates agreed in a meeting live in an email thread, and nobody notices when they pass.

With SparkFlow: deadlines and status sit on the record, and actions in progress and their owners show on the dashboard.

Closing without checking

The action is marked done, but nobody confirms the fix is in place or that it worked.

With SparkFlow: route closure through an approval path so someone signs it off on the record.

Evidence scattered across tools

The finding is in one file, the photos in another and the approval in an inbox, so the trail is hard to prove.

With SparkFlow: finding, evidence, action and approval stay linked on one record.

No view across audits

The same issue appears at three sites and nobody sees the pattern until the certification audit does.

With SparkFlow: dashboards show findings by type and site, so patterns are visible early.

Correction and corrective action are not the same thing. A correction deals with the nonconformity itself, such as replacing a missing record or repairing a guard. A corrective action deals with the cause, so it does not happen again. A good response to a finding usually needs both, and SparkFlow lets you record each against the same finding.

Dashboards

See audit findings and actions across every site

A finding is only useful if someone follows it through. Dashboards turn individual audits into a live picture for HSE managers and leadership, ready for management review.

  • Findings by type, site and audit.
  • Open actions, who owns them and which are overdue.
  • Closed findings with the evidence ready for an auditor.
  • Repeat findings that point to a weakness in the system.

Who Uses It

One audit record, four ways of using it

HSE managers

  • Set up audits and approval paths
  • Track open findings across sites
  • Generate audit reports on demand

Auditors

  • Record findings and evidence on site
  • Follow findings through actions and approvals
  • Check that closures were verified

Action owners and supervisors

  • See the actions assigned to them
  • Update progress and attach evidence of the fix
  • Stop chasing instructions by email

Leadership

  • Review results on a live dashboard
  • See which findings keep returning
  • Prepare for management review with current data
Audit Practice

Before, during and after an HSE audit

A practical checklist for each stage. It follows the approach in ISO 19011, the guidance standard for auditing management systems, and works whichever tool you use to record the results.

Before the audit
  • Confirm the objectives, scope and criteria.
  • Choose auditors who are competent and independent of the area being audited.
  • Review the results of the previous audit and any open actions.
  • Share the audit plan with the people being audited.
  • Prepare the audit form and the approval path.
During the audit
  • Hold an opening meeting to confirm scope, timing and ground rules.
  • Sample records and observe the work, not just the paperwork.
  • Talk to people at different levels, including frontline workers and contractors.
  • Record each finding with its evidence as you go.
  • Hold a closing meeting so there are no surprises in the report.
After the audit
  • Classify the findings using your agreed categories.
  • Agree corrective actions with an owner and a deadline for each.
  • Route findings and closures for approval.
  • Issue the report to management.
  • Verify that actions were completed and that they worked.
Sample Audit Questions

Sample HSE audit questions to start from

Use these as a starting point and adapt them to your sites, your risk assessments and the standard you are audited against. In SparkFlow, each question becomes a line on a configurable audit form.

Sample audit questions: hazard identification and risk assessment

  • Is there a documented process for identifying hazards?Look for: Procedure, hazard register
  • Are risk assessments in place for routine and non-routine activities?Look for: Dated assessments for sampled tasks
  • Are assessments reviewed after incidents, changes and at set intervals?Look for: Review dates, revision history
  • Are workers involved in identifying hazards?Look for: Consultation records, toolbox talk notes
  • Are controls applied in line with the hierarchy of controls?Look for: Control measures listed, site observation
  • Are residual risks rated and accepted by someone authorised?Look for: Sign-off on the assessment
  • Do workers know the hazards and controls for their task?Look for: Interviews at the work area
  • Are changes assessed for risk before they are made?Look for: Change records, assessments

Any question answered "no" or "partly" becomes a finding, and each finding that needs action becomes a corrective action with an owner and a deadline.

Sample audit questions: emergency preparedness and response

  • Are plans in place for foreseeable emergencies such as fire, spill and medical events?Look for: Emergency plans, site maps
  • Are emergency roles such as fire wardens and first aiders assigned and current?Look for: Role lists, certificates
  • Are drills held at planned intervals?Look for: Drill schedule and records
  • Are drill results reviewed and lessons acted on?Look for: Review notes, linked actions
  • Is emergency equipment maintained and tested?Look for: Inspection and test records
  • Are contractors and visitors covered by the arrangements?Look for: Induction content, visitor briefings
  • Are emergency contact details current?Look for: Contact lists, notice boards
  • Are plans reviewed after drills and real events?Look for: Revision history, review dates

Any question answered "no" or "partly" becomes a finding, and each finding that needs action becomes a corrective action with an owner and a deadline.

Sample audit questions: contractor management

  • Are contractors assessed on HSE performance before work starts?Look for: Pre-qualification records
  • Are HSE requirements stated in contract documents?Look for: Contract clauses, scope documents
  • Are contractor inductions completed and recorded?Look for: Induction records for sampled workers
  • Are contractor risk assessments and method statements reviewed before work?Look for: Reviewed and approved documents
  • Are work permits issued where the work requires them?Look for: Permit records for sampled jobs
  • Are contractors inspected or audited while they work?Look for: Inspection and audit records
  • Are contractor incidents reported and investigated?Look for: Incident reports and investigations
  • Is contractor performance reviewed when the work ends?Look for: Evaluation records

Any question answered "no" or "partly" becomes a finding, and each finding that needs action becomes a corrective action with an owner and a deadline.

Sample audit questions: incident investigation and corrective action

  • Are incidents and near misses reported?Look for: Reporting records, trend data
  • Is the depth of investigation proportionate to severity?Look for: Investigation records for sampled events
  • Are root causes identified, not just immediate causes?Look for: Cause analysis on the record
  • Does every corrective action have an owner and a deadline?Look for: Action register
  • Are actions completed on time?Look for: Completion dates against deadlines
  • Is the effectiveness of actions checked?Look for: Verification or review notes
  • Are lessons shared with other sites and teams?Look for: Briefings, bulletins
  • Are records kept as required?Look for: Retention evidence, linked records

Any question answered "no" or "partly" becomes a finding, and each finding that needs action becomes a corrective action with an owner and a deadline.

Audit Programme

How to plan an HSE audit programme

An audit programme is the set of audits planned for a period, each with its scope, criteria, schedule and auditors. ISO 45001 asks for audits at planned intervals, and the plan should reflect what matters most to your organisation.

Risk

Audit high-risk activities, sites and contractors more often than low-risk ones.

Previous results

Revisit areas with repeat findings or overdue actions until they are stable.

Change

Add audits when you open a site, change a process, bring in new equipment or take on new contractors.

Obligations

Allow for certification cycles, client requirements and legal duties that fix when audits must happen.

Audit programme checklist

  • List every site, process and contractor in scope
  • Set objectives, scope and criteria for each audit
  • Set the frequency from risk and previous results
  • Assign competent auditors who are independent of the area
  • Allow enough time and resources for each audit
  • Tell the people being audited what to expect
  • Record results, findings and actions for every audit
  • Review the programme against results and in management review

The programme is a management decision. SparkFlow holds the records that each audit produces, so you can review the programme against real findings and actions instead of memory.

Audit Report

What an HSE audit report should include

A clear report makes it easy for management to decide and for action owners to act. These are the elements most audit reports cover.

Core audit report contents

  • Audit objectives and scope
  • Audit criteria: the standard, legal requirement or procedure used
  • Dates and locations of the audit
  • Audit team and the people interviewed
  • Findings, each with its category
  • The evidence behind each finding
  • Good practice worth recognising
  • Audit conclusions
  • Agreed corrective actions, owners and deadlines
  • Approvals and the date of follow-up

In SparkFlow the report is generated from the audit record, so the findings, evidence and actions it lists are the same ones the team is working from.

ISO 45001

Where an ISO 45001 audit looks, and where the evidence sits

Auditors sample across the whole management system. This table shows the clauses most audits spend time on, what is usually checked and where the matching records live in SparkFlow.

ISO 45001 clauses, what auditors check and where evidence sits in SparkFlow
Clause What an auditor checks Evidence usually sampled In SparkFlow
6.1 Hazards and risks are identified and controls are decided. Risk assessments, hazard registers Risk & CAPA records
8.1 Controls are applied in the work, including for contractors. Permits, inspection records, procedures Permit to Work and inspection records
8.2 Emergency plans exist, drills are held and lessons are acted on. Plans, drill records, review notes Audit and inspection forms, with actions for gaps
9.1 Performance is monitored, measured and compliance is evaluated. Inspection results, incident trends, KPIs Live dashboards
9.2 Internal audits are planned, impartial, reported and followed up. Audit programme, reports, findings Audit workflows and reports
10.2 Incidents and nonconformities are investigated and causes are fixed. Investigations, corrective actions, closure Incident reporting and Risk & CAPA

Clause numbers refer to ISO 45001:2018. Check them against your own copy of the standard and your certification body's guidance, because audit scope and sampling differ between organisations.

Audit Readiness

The evidence auditors ask for, already connected

Auditors sample records. The easier it is to follow a record from the event to the fix, the easier the audit. Because SparkFlow keeps these on one platform, you can trace the thread without opening five tools.

Inspection records

Dated checklists, failed items and photos, with the action that followed each one.

Safety inspections

Incident investigations

Reports, root causes and corrective actions linked on one record.

Incident reporting

Corrective action closure

Owners, deadlines, status and proof that each action was closed.

Risk & CAPA

Permit records

Permits and approvals, so work authorisation can be checked against what was done.

Permit to work

Software does not pass an audit for you. What it gives you is organised evidence: dated records, a named owner for every finding and proof that each one was closed, so preparing for a certification, client or regulator audit means reviewing data instead of searching for it.

Standards and UAE Context

HSE audit requirements for UAE employers

What you audit against depends on your sector, emirate, trade licence and the certifications you hold. These are the frameworks UAE teams most often meet.

ISO 45001

Clause 9.2 requires internal audits of the OH&S management system at planned intervals, with impartial auditors and results reported to management.

ISO 14001 and ISO 9001

Both also expect internal audits at planned intervals, so one audit process can serve safety, environment and quality systems.

Abu Dhabi: OSHAD-SF

Abu Dhabi workplaces follow the OSHAD System Framework, supervised by the Abu Dhabi Public Health Center.

Dubai, federal and free zones

Federal rules are enforced through MOHRE, Dubai applies municipal and authority requirements, and free zones add their own regimes.

What this means for your audit programme: keep an audit programme with defined scope and criteria, use competent and impartial auditors, report results to management, act on every nonconformity and keep the records. Those are the same expectations whichever framework applies to you.

SparkFlow forms, routing and approval paths are configurable, so you can match them to your own procedures and to the framework that applies to each site. Requirements change, so confirm exact obligations with your regulator, certification body or HSE adviser.

Buyer Guide

What to look for in HSE audit software

Use this list when you compare tools. Good audit software makes recording a finding easy for the auditor and ignoring it hard for everyone else.

  • Configurable audit forms that match your standard or procedure.
  • Mobile capture of findings and evidence on site.
  • Findings linked to corrective actions with an owner and deadline.
  • Approval workflows for findings and closures.
  • Reports generated from records, not written by hand.
  • Live dashboards with a full history of who did what and when.
  • Links to inspections, incidents, permits and risk, so audit evidence is not split across tools.
FAQ

HSE audit software FAQs

HSE audit software runs internal and external audits as structured digital workflows. Auditors record findings and evidence against each audit question, every finding is linked to a corrective action with an owner and deadline, findings and closures go through an approval path, and the audit report is generated from the records instead of written by hand.

An inspection checks conditions and behaviour at a point in time, such as a blocked exit or missing PPE. An audit checks whether the HSE management system works as intended, by comparing practice and records against criteria such as ISO 45001 or your own procedures. Inspections are one source of evidence for an audit, but they are not the same activity.

An internal audit is carried out by or for the organisation itself, to check its own management system. External audits are carried out by someone outside it: a customer or client auditing a supplier or contractor, or a third party such as a certification body or regulator. SparkFlow records both kinds of audit on the same findings and action process.

A nonconformity is a failure to meet a requirement, whether that requirement comes from a standard such as ISO 45001, a legal duty or your own procedure. Many organisations grade them as major or minor, and also record observations and opportunities for improvement. The exact definitions and thresholds depend on your procedure or certification body.

A compliance audit checks whether the organisation meets its legal requirements and the other obligations it has signed up to, such as client or industry requirements. ISO 45001 clause 9.1.2 also expects an organisation to evaluate compliance with legal and other requirements. It sits alongside management system audits, and both can be recorded on the same findings and action process in SparkFlow.

ISO 45001 clause 9.2 requires internal audits at planned intervals, run to an audit programme with defined criteria and scope. Auditors must be objective and impartial, results must be reported to relevant management, action must be taken on nonconformities, and documented information must be kept as evidence that the programme was carried out.

There is no single legal answer. ISO 45001 asks for audits at planned intervals, which should reflect the importance of the processes and the results of previous audits. Many organisations cover each site and process at least once a year and audit high-risk areas more often. Set the programme from your risk assessment and the requirements that apply to you.

An audit programme is the set of audits planned for a period, with the scope, criteria, schedule, auditors and resources for each. Plan it from risk, the results of previous audits, changes to sites, contractors or processes, and any certification or client requirements, so high-risk areas and repeat problems are audited more often. SparkFlow holds the records each audit produces, so the programme can be reviewed against real results.

A typical report states the audit objectives, scope and criteria, the dates and locations, the audit team and the people interviewed, the findings with the evidence behind each one, and the audit conclusions. For nonconformities it should also show the agreed corrective actions, owners and deadlines. SparkFlow generates the report from the audit records.

Audit evidence is the records, statements and observations an auditor uses to decide whether a requirement is met. It includes documents and records such as inspection results, investigations and permits, interviews with people at different levels, and direct observation of the work. In SparkFlow, evidence such as photos and observations is attached to the finding it supports.

Raise an action directly from the finding and assign an owner and a deadline. The action is tracked to closure in SparkFlow's Risk & CAPA workflow, and the finding, evidence, approvals and closure stay linked on one record, so you can show an auditor how each finding was resolved.

Agree the action with the person responsible, give it an owner and a deadline, complete it, and then have someone verify that it was done and that it worked before the finding is closed. Closing without verification is a common reason findings repeat. In SparkFlow the action, the evidence of the fix and the approval of closure stay on the same record as the finding.

A correction deals with the nonconformity itself, for example replacing a missing record or repairing a guard. A corrective action deals with the cause, so the problem does not come back, for example fixing the process that allowed the record to go missing. A good response to a nonconformity usually includes both.

Yes. Auditors can record findings, observations and photo evidence on site from web and mobile, instead of writing notes on paper and typing them up afterwards.

Yes. Forms, routing and approval paths in SparkFlow are configurable, so each audit type can follow your own checklist, hierarchy and procedure. Confirm with your certification body or HSE adviser that the questions and finding categories you configure match the standard you are audited against.

It keeps the evidence an auditor asks for in one place: dated audit and inspection records, incident investigations, a named owner for every finding and proof that each action was closed. Reports come from live records, so preparation is a review of the data rather than a search through folders and spreadsheets. It does not replace the audit itself or guarantee a result.

HSE managers who run the audit programme, internal and external auditors who record findings, action owners and site supervisors who fix them, and leadership who review results. Each role sees the part of the audit process that is theirs.

Yes. SparkFlow is built in Dubai for UAE HSE teams, and its forms and approval paths are configurable to match your sector and internal procedures. Requirements differ between federal, Abu Dhabi (OSHAD-SF), Dubai and free zone rules, so confirm what applies to your licence with your regulator or HSE adviser.

Yes. SparkFlow runs audits, inspections, incident reporting, permit to work and risk and CAPA on one platform, so the records an auditor samples and the actions that follow stay connected in one system.

SparkFlow supports HSE teams in oil and gas, manufacturing and construction, where audits, contractors and corrective actions need to be tracked across multiple sites.
